			<content:encoded><![CDATA[<p>Washington, February 2, 2009 &#8212; &#8220;The state of America&#8217;s health care is poor,&#8221; Jeffrey P. Harris, MD, FACP, president of the American College of Physicians (ACP), reported today at the annual State of the Nation&#8217;s Health Care briefing. &#8220;There are too many uninsured and underinsured people. We have too few primary care physicians.&#8221;<br />
	&#8220;The problems are big, so the solutions must also be big,&#8221; Dr. Harris declared. <span id="more-79"></span>At the briefing, ACP provided recommendations on how President Obama and Congress can take immediate, sustained, and dramatic steps to provide affordable and accessible health care to all Americans and provide every American with access to a primary care physician.<br />
	The Institute of Medicine (IOM), a private, non-governmental organization, recently announced that the U.S. needs 16,000 more primary care physicians just to meet the needs of currently underserved areas. The shortage will grow to 40,000 or more physicians, assuming current rates of <a href="http://www.findcheaphealthinsurance.net/">health insurance</a> coverage, according to two recent studies. At a time when America&#8217;s aging population is increasing, the demand for general internists and family physicians will continue to grow at a much faster rate than primary care physician supply, in the absence of policies to immediately and substantially increase primary care workforce capacity.<br />
	&#8220;Giving all Americans an insurance card will not guarantee that everyone will have access to care,&#8221; Dr. Harris emphasized. &#8220;There are not enough primary care physicians to care for them.&#8221;<br />
	To underscore the importance of the timing, Dr. Harris added: &#8220;Given the fact that it takes a minimum of seven years to train a primary care physician (medical school and residency combined), the U.S. cannot afford to delay implementation of policies to attract more new physicians to primary care and to sustain those already in practice.&#8221;<br />
	Therefore, policies to expand primary care workforce capacity need to be implemented immediately and go hand-in-hand with coverage expansions. We need to act now to influence the career choices of medical students and physicians already in residency programs, and the retirement and career decisions of primary care physicians already in practice.&#8221;<br />
	The ACP recommendations also call for:<br />
- Setting specific goals for increasing primary care, including policies to achieve them and measures to evaluate their success;<br />
	- Reforming primary care payment policies to achieve market competitiveness with other specialties, including immediate and sustained increases in Medicare fee-for-service payment;<br />
	- Expanding the Patient-Centered Medical Home, and<br />
	- Issuing an Executive Order by President Obama to assure that all federal agencies are working together to set primary care workforce goals and the policies necessary to achieve them.<br />
	ACP&#8217;s briefing cited several examples of how patients will suffer if health care reform does not expand the primary care physician workforce capacity at the same time as coverage is expanded:<br />
	- For the newly insured, there will be long wait times to get an appointment with a primary care physician, if they are able to find one at all.<br />
	- In a growing number of communities, it may become impossible for people who do not currently have a relationship with a primary care physician to find an internist, family physician or pediatrician who is taking new patients. Not because established primary care physicians do not want to accept the newly-insured into their practices, but because they have no time left in an already over-scheduled day to take on any additional patients.<br />
	- Patients will have less time with their primary care doctor. Despite insurance coverage, without changes in the way care is provided, physicians may have to further decrease the time they currently spend with patients in order to try to accommodate increased demand for services associated with covering more Americans &#8212; which could have a negative impact on quality, access, and timeliness.<br />
	- Massachusetts&#8217; experience is a case in point of what can happen if coverage is expanded without expanding the primary care workforce. When health <a href="http://www.findcheaphealthinsurance.net/">insurance coverage</a> was recently expanded to nearly 95 percent of the state&#8217;s residents, some low income residents reported difficulty finding a physician or getting an appointment. In fact, the wait to see primary care physicians in Massachusetts has reportedly grown to as long as 100 days.<br />
	ACP&#8217;s recommendations are supported by the 126,000 member organization&#8217;s new comprehensive evidence review, How is a Shortage of Primary Care Physicians Affecting the Quality and Cost of Medical Care? Using more than 100 studies, the paper notes that &#8220;even though decades of research tell us that primary care is the best medicine for better health care and lower costs, the current U.S. health care system fails to support policies and payment models to help primary care survive and grow.&#8221;<br />
	A paper published by the ACP in the Annals of Internal Medicine that compared U.S. health care with care in other industrialized countries found that the best performing health care systems abroad, unlike the United States, are built on a strong foundation of primary care.<br />
	&#8220;A better health care system must result in everyone having health insurance coverage, and everyone having access to a primary care doctor. Anything less than that will fail to provide Americans with access to affordable, comprehensive and personal care they need and deserve,&#8221; Dr. Harris said in concluding ACP&#8217;s State of the Nation&#8217;s Health Care briefing.<br />

<p>We know what you&#8217;re thinking, &#8220;Why do I need health insurance? I&#8217;m young, I&#8217;m healthy, and doctor visits are few and far between. So why pay for something I&#8217;ll never use?&#8221; Hey, we understand where you&#8217;re coming from. But accidents and illnesses happen without warning, even to the strapping young adults such as you. Sure, <a href="http://www.findcheaphealthinsurance.net/">health insurance</a> is expensive, but not having it will cost you dearly.</p>
<p>First things to know</p>
<p>Let&#8217;s get one thing straight, health care in the United States is a nightmare, few will argue that. There are thousands of options when it comes to receiving care and paying for it, some of them good, some of them not so much. Deductibles range from a few hundred dollars up to $1,000 or more, depending on your policy. Also, indemnity plans require a co-payment on medical care; meaning you&#8217;ll be responsible for a percentage of the treatment costs along with your deductible. Generally, indemnity plans pay only for accidents or illness; they usually don&#8217;t cover preventative care.</p>
<p>Managed care is the complete opposite of indemnity coverage. Deductibles are usually smaller, co-payments are lower, and preventative care is usually covered. Your options, however, are limited. Through a managed care plan, you can only choose between health care providers who are contracted by your health maintenance organization. If you go elsewhere, you pay, the full amount. Since that&#8217;s a pretty rough deal, many managed care plans are offering hybrid options that include many of the desirable characteristics of an indemnity plan.</p>
<p>Which way to go </p>
<p>If you find a job that offers health insurance and you&#8217;re single, take it. It may not be perfect, but it beats anything you can find on your own. When you sign up through your employer, you&#8217;ll probably be confronted with many options. Take a good, long look at them and ask for help from a human resources representative if need be, but make sure you choose the plan that&#8217;s right for you. Chances are, if you&#8217;re young and healthy,  you&#8217;ll want a plan with a low premium and higher deductible. Look for a plan that minimizes your out of pocket expenses. This indemnity policy offers extremely low premiums, but deductibles can be very high up to $2,500. <p>Medical insurance for employees of small businesses is a hot button issue. Approximately 45 million U.S. residents do not have health insurance. Of those, about 20 million <span id="more-77"></span>are employees, or a family member of an employee; of a business that has 50 or fewer employees. For companies that have more than 50 employees, more than 96% of workers have medical insurance. That number is only about 43% for small businesses.</p>

<p>Usually, health insurance companies are allowed to ask questions about your medical history and to decline to insure you if you are suffering from a current medical problem or have a history of particular conditions. More usually however insurance companies will offer to cover you, although they will either keep out specific conditions from your cover or raise your premium and cover these conditions. In Florida however there is an exception to this rule in the case of a history of breast cancer if you have finished a course of treatment for the condition at least two years prior to your current application for insurance. In cases where this applies an insurer is not allowed to deny you insurance. </p>
<p>When you have been covered by a group insurance plan for at least three months and later lose your cover then, within Florida, you can buy an alternated plan and an insurance company have to offer you the choice of a minimum of two policies. Moreover, companies cannot impose a new exclusion for pre-existing conditions. However, they are allowed to enforce an exclusion clause where you have not completed any previous qualifying period. </p>
<p>Where you do not meet the qualifications for a conversion plan but are HIPAA entitled then once again an insurance company must not refuse to cover you and must once again offer you a choice of a minimum of two policies. To be HIPAA entitled you will need to have had a minimum of eighteen months of worthy and continuous coverage (the final day of which has to have been under a <a href="http://www.findcheaphealthinsurance.net/">group health insurance</a> policy) and have used up any suitable COBRA or continuance coverage. Additionally, you cannot presently have any health insurance (or be covered by a group policy that is about to expire) and cannot be eligible for a further group policy or for Medicaid or Medicare. An application for cover on the basis of HIPAA eligibility needs to be ended within 63 days of losing your previous cover. </p>
<p>In cases where an insurer or HMO can no longer give cover, because they have for instance become insolvent or you have moved to a location out of their service area, then other insurance companies are required to offer to provide you with cover whatever your state of health. </p>
<p>Newborn children, adopted children and children placed for adoption must be covered under the terms of a parent&#8217;s <a href="http://www.carinsurance-site.com/">individual insurance</a> plan for 31 day from the date of birth, adoption or placement. </p>

At the beginning of this year, the Spokane-based <span id="more-74"></span>nonprofit employers group began offering to members a health-insurance plan provided by United Healthcare, ending a 50-year relationship with Premera Blue Cross, says Jim DeWalt, the organization&#8217;s president. Since then, Associated Industries has picked up about 80 additional companies as members, 70 of which have signed up to have the Spokane group offer their employee health benefit package, DeWalt says. </p>

<p>The shift to a new insurer, he says, has re-energized the nearly 100-year-old association.<br />
&#8220;We had a decline (in membership) related to a sharp decrease in medical health participation,&#8221; DeWalt says.<br />
Associated Industries now has 535 member companies. Of those, nearly 250 companies buy employee health coverage through the Spokane group&#8217;s Associated Employers Trust, which now covers 5,980 people, counting both employees and their dependents, up from 3,510 people at the beginning of the year. At its peak six years ago, the trust was covering 313 member companies and more than 8,000 people.<br />
The trust is expected to collect $20 million in health-care premiums this year from its member employers, up from $14 million in 2007, DeWalt says.<br />
The gains in membership and health-care plan participation are both directly related to Associated Industries&#8217; decision to switch to UnitedHealthcare, which is a division of Minnetonka, Minn.-based UnitedHealth Group, DeWalt says.<br />
The switch helped Associated Industries expand its membership base in two ways. First, Premera had 60 percent of the insurance market share in Washington, both through direct sales and through its relationship with other associations, limiting Associated Industries&#8217; opportunity to market the product, says Ted Blotsky, Associated Industries&#8217; senior vice president. He says the Spokane organization had no ability to compete for established Premera customers. Since UnitedHealthcare is new to the small-business market, it offers a new competitive edge to previously limited choices, he says.<br />
&#8220;With a company that has only 10 percent of the market it opens up our ability to market the health plans,&#8221; Blotsky says.<br />
Secondly, UnitedHealthcare provides Associated Industries with plans that can be offered to companies that have five to 99 employees. Premera&#8217;s plans only could be offered to companies with 50 or fewer employees.<br />
&#8220;Over time, our needs were different than what could be met by Premera Blue Cross of Washington,&#8221; DeWalt says.<br />
To continue growing in the future, the association can sell UnitedHealthcare benefits outside of Washington, which it could not do with Premera Blue Cross&#8217; plans, Blotsky says.<br />
DeWalt says UnitedHealthcare hadn&#8217;t previously offered health plans to small businesses, and has agreed to limit the number of associations it deals with in the small-business market to three, giving Associated Industries less competition to market plans statewide than it faced when offering Premera&#8217;s plans.<br />
That, in turn, is helping the employers group to expand its statewide footprint in other ways, by selling its other services to the companies that sign on for the health-care plans. In addition to health plans, Associated Industries offers electronic payroll, training, and other human-resources services. DeWalt says the robust response to the new health offering gives the association greater confidence that it can sell its services statewide.<br />
Blotsky says 50 percent of the new business it&#8217;s now doing through its Associated Employers Trust is from the Interstate 5 corridor in Western Washington. The nonprofit has added two employees in the last year, and expects to hire two more staff members to help it handle the growth and expected increases in other services it offers.<br />
DeWalt says participation in the association&#8217;s health plans had dwindled in recent years partly because Premera&#8217;s plans didn&#8217;t allow Associated Industries to continue to cover member companies when they grew beyond 50 employees.<br />
Seeing UnitedHealthcare enter the market was a welcome addition, he says. Before that, only Premera Blue Cross and Asuris Northwest Health were offering small-group plans to associations, he says.<br />
&#8220;If you believe in a free market, you&#8217;d believe that&#8217;s not a healthy environment,&#8221; DeWalt says.<br />
New approach<br />
DeWalt says UnitedHealthcare&#8217;s different approach to marketing has helped it regain momentum.<br />
Rather than flooding the state by offering its new product to a number of similar associations, UnitedHealthcare is collaborating with Associated Industries to offer a new product that gives it a competitive boost in the state, with the possibility of future expansion into Idaho, Oregon, and Montana - something that was not possible with Premera, Blotsky says. </p>
